You need to call the nurse or physician to update them on the changes, and to develop a care plan that will meet the needs and the goals of the person, family, and community members who are caring for the person. You know that you need to gather information by observing and interacting with the person. You also know that you will need to talk with the family and the community members who are involved to understand their needs and their desire to keep the person at home.
