SCENARIO
A 25 year old male patient was brought in ER by barangay tanod in semi-conscious, unresponsive condition with multiple injuries and bleeding in the head, face, chest, abdomen and lower extremities after being hit by a water delivery van (road accident) and was thrown 20 meters from the barangay outpost.
On admission, the vital signs taken, observation done and recorded:
BP- 80/60, Pulse rate-120 bpm, Respiration- shallow, rapid 110 cpm, Temperature-38.1C, GCS-9 (EO-3, MR-3, VR-3), SPO2-85%. Sustained lacerated wound right parietal area, with periorbital edema, bleeding in the nose, mouth, suspected rib fracture, bloated bruised abdomen, and deformed right arm and right lower leg.
Fifteen minutes later, panicking relatives came in and the Police of Bacoor City arrived for investigation in the medico-legal case. History revealed that the patient was going to his work in a nearby bakery using a borrowed bike from his neighbor when the incident happened. Relatives stated that the patient had a history of pernicious anemia, bronchial asthma, PTB since a year ago and a CKD case taking medications. An alcoholic and a chain smoker for 5 years. One of the witnesses saw the van speed up to the Coastal direction and nowhere to be found. A bystander witnessed the scene and was brought to the police station.
