Heparin protocol Heparin drip: 25,000 units in 500 mL D5W Bolus: 80 units/kg Starting infusion: 18 units/kg/hour Titrate according to the following: PTT < 45 seconds: Bolus with 40 units/kg and increase rate by 3 units/kg/hour PTT 45-48 seconds: Bolus with 40 units/kg and increase rate by 2 units/kg/hour PTT 49-66 seconds: No bolus and increase rate by 1 unit/kg/hour PTT 67-109 seconds: No bolus and no change in rate PTT 110-130 seconds: No bolus and decrease rate by 1 unit/kg/hour PTT > 130: No bolus, Stop infusion for 1 hour, decrease rate by 2 units/kg/hr e. Pt. weight is 250 lb. calculate the amount you would give for the bolus. f. Calculate the beginning infusion rate. What will you set the pump at? 8 The PTT after 6 hours is 47 seconds. What will you do? h. Calculate the bolus and new infusion rate.
