Samantha is seeking treatment due to symptoms of depression and relationship disturbance. She is presenting for treatment largely at the urging of her husband. Years ago, she moved from the Midwest to our area as her husband secured a new job, and was also adjusting to her role as a new mother. Prior to both life changes, Samantha had a thriving business as a real estate agent. Both the move and the shifting role as a stay-at-home mother caused her to feel somewhat lost, frequently becoming intensely dysphoric and anxious about the future. Years have transpired since the move, and she feels pressure to do something with her life, now that her son is in school. But this topic is met with intense anxiety and defensiveness, expressing much anger at the question, "what do you do when your son is at school?" Recently, she (with her husband's knowledge and acceptance) has pursued amateur pornography, engaging in relationships with men on the internet; much of her time and energy during the day is spent on this activity. She expresses feeling empowered by these activities, citing that she feels more comfortable with her body than she ever has, and she has enjoyed exploring her sexuality. While her husband has accepted this, he is concerned about her lack of direction in life, as well as her chronic feelings of insecurity and emptiness.
Additional relationship discord has emerged as she feels abandoned and endlessly “let down" by her husband. When she feels disappointed by him, she is prone to frantic dysphoric episodes, in which she lies on the floor in the basement, crying and arguing with her husband. Her complaints about her husband largely involve his "mishandling" of her depression, and what she perceives to be thoughtlessness about her feelings. Her husband has expressed fear of her during these episodes, and he tries desperately to avoid "setting her off." Samantha is unsure of her goals in treatment. In session, she indicates that she largely sees her husband as the problem; she defends much of her behavior as merely reactive to others' behaviors, and fails to accept much responsibility for the ways in which her life is not working.
