When the Risk Priority Number (RPN) on the Failure Modes & Effects Analysis (FMEA) is over 750
When the solution requires different subject matter expertise than required earlier in the project
When interpersonal relationships are experiencing difficulty during team meetings
Before constructing the "As Is" process map with the improvement team
