Patient History: A person has been
referred to an oncologist with an abnormally high white blood cell
count. Flow cytometry analysis of a blood samples reveals they have
very high levels of large malignant B cells in circulation.
Molecular profiling shows that these B cells have a mutation in the
p53 gene, which is associated with a poorer patient prognosis.
These malignant B cells express the CD19 antigen, like all B cells,
but they also express gB antigen from the Epstein Barr virus, which
may be oncogenic.
Diagnosis: The patient is diagnosed with
diffuse large B cell lymphoma (DLBCL). They are initially treated
with chemotherapy, followed by a stem cell transplant from the bone
marrow tissue bank. They develop graft versus host disease (GvHD)
and are treated with immune-suppressive drugs.
