OPERATION: BronchoscopyANESTHESIA: Infiltration of 10mL of 0.1%
Xylocaine
PREOPERATIVE MEDICATIONS: Demerol 75mg and 0.6mg of atropine.
The patient also received a total of 4mg of Versed intravenously
just prior to and during the operation.
PROCEDURE: The bronchoscopy was performed. A nasotracheal
tube was inserted into the right nostril while the patient was
receiving the supplemental oxygen through the left nostril, and the
saturation, which was being monitored closely, did not fall below
94% at any time.FINDINGS: The epiglottis was normal. The vocal
cords were normal, with normal closure on phonation. The trachea
was normal without endotracheal lesion or external tracheal
compression, and the carina was sharp and clear.
RIGHT LUNG: The right mainstem bronchus, the bronchus of the
right upper lobe, right lower lobe, and right middle lobe, with all
segments and subsegments identified. There were moderate
whitish secretions in the right lower lobe bronchus, which were
suctioned clear. There was no endobronchial lesion or external
bronchial compression seen in the right lung.
LEFT LUNG: The mid to lower section of the left mainstem
bronchus was displaced anteriorly and medially; this was also
partly occluded by a combination of an external bronchial tumor,
which was eroded into the left mainstem bronchus. There was a 50%
obstruction of the left mainstem bronchus, but the bronchoscope
could be passed beyond the obstruction and the left lower lobe
bronchus, with all segments and subsegments identified.
Multiple biopsies, washings, and brushings were obtained from
the left mainstem bronchus tumor. There was some bleeding around
the tumor, even before the bronchoscope was advanced to the tumor,
which was washed.
