patient, Diana G. Pike, while also listening to her supervisor,
Emily, giving her instructions about something unrelated. Mrs. Pike
filled out the new patient registration form and mentioned to Cindy
that she is a new patient. Mrs. Pike has a seat, and Cindy is
completing her registration. She types in the last name of Pike and
sees that there is a Diana Pike in the system. Diana G. Pike has a
date of birth of 9/18/1957 and lives at 367 Elmwood Drive,
Carollton, GA. Diana Pike has a date of birth of 8/18/1957 and
lives at 387 Elmhurst Drive, Carollton, GA. She assumes someone
inaccurately entered Diana Pike into the system, and enters
information about today’s appointment under Diana Pike’s record in
the master patient index.
