You are reviewing orders for your post-surgical male patient (80
kg) who is receiving heparin 20,000 units in D5W 500
ml at a rate of 50 ml/hr who has recently undergone vascular
surgery. The smart IV pump will only deliver heparin in
units/kg/hr. The maximum allowable starting dose is 18
units/kg/hr.
The maximum allowable dose is 18 units/kg/hr and the physician
accepts your dosing adjustment recommendation
(Good Job!). In discussing the dose adjustment with the
physician, she is wants to be careful about the total amount of
fluid the patient is getting and wants to ensure the patient is
getting less than 1500 ml/hr in his IV medications per
day. The following is the list of medications. How many total
milliliters of fluid is the patient getting per day?

Heparin 18 units/kg/hr
Vancomycin 1000 mg in NS 250 ml q12hrs x 3 doses
Piperacillin/Tazobactam 4.5 gm in 100 ml q8hrs x 3 dos
